About this document
A child travel consent records that a parent or guardian agrees to a child making a journey with another adult, or alone, and to that adult making day-to-day and medical decisions while away. It sets out the journey, where the child will stay, who is paying, contact details and the child's health information.
This blank form is completed by hand, with signature lines for both parents or guardians. Airlines, border officials and countries have their own rules on what they ask for, and may require a particular wording or notarization, so check with the carrier and the destination before you travel.
