About this document

A child medical consent lets a parent or guardian authorise another adult, such as a grandparent, caregiver or family friend, to arrange and agree to medical, dental, surgical and mental-health care for their child if the parent cannot be reached in time.

It is a blank form to complete by hand. It records the child's medical details, insurance and your contact numbers, and asks to be contacted first for anything other than urgent care. Whether a provider accepts it, and what it needs to be signed, witnessed or notarized, varies by state and by provider.

Questions

About the child medical consent

  • Not necessarily. Providers and states differ on what they accept. It is sensible to check with the child's doctor and to keep a copy with the child's insurance details.
