About this document
An offer letter tells a candidate that they are being offered a position and sets out the main terms: title, start date, hours, pay, any variable pay, benefits, time off and the conditions the offer depends on. It says how long the offer stays open, how to accept and who to contact with questions.
This blank form is a letter to complete by hand. It states that it sets out the main terms and that the employment agreement the person is asked to sign governs if the two differ. It is not a contract of employment by itself. Which checks an employer may require, what may be asked of a candidate and what an offer commits the employer to vary by state, so check what applies.

Questions
About the offer letters
That depends on its wording and the state. The form says the agreement the person signs governs the employment if it differs. If the details matter, have a lawyer look at it.
