About this document

A maintenance request form gives a tenant one place to tell a landlord what needs fixing: when they are asking, who they are, where on the property the problem is and what it is. It also gives the landlord a section for internal use to note whether it was an emergency, whether the work was done and when.

This builder asks whether the property is residential or commercial, its address and unit, the phone number a tenant should call in a maintenance emergency with optional backup contacts, and who is leasing the property out. The request details and maintenance details are left as blank lines for the tenant to complete by hand.

The form does not say how quickly a landlord must respond, what counts as an emergency, or what a tenant may do if a request is not acted on. Those depend on the lease and your state.
