About this document
An activity waiver form is a short sheet a participant completes before joining an activity. It collects health and dietary information, an emergency contact, permission for emergency treatment and the participant's acknowledgement of the risks and release.
It is a blank to complete by hand and is shorter than a full liability waiver and release. It has a place for a parent or guardian where the participant is a minor and one for the organizer's signature.
