About this document
A complaint letter sets out a problem with a product or service in order: what you bought, what went wrong, what you have already done about it, what it has cost you and what you ask for. A well-organised letter makes it easier for a business, card provider or agency to deal with.
This blank form is a letter to complete by hand. It states what you intend to do if there is no reply, such as taking it to a complaints scheme or consumer agency, and reserves your rights. It does not say what remedy you are entitled to, which depends on the law and on what you bought.
