Key features
For you or for someone else
Say who the affidavit is about. For someone else it records where they live, since when and how you are related.
For a person who has died
Their last address, how long they lived in the state, their date of death and your relationship to them.
Where and for how long
Your address, since when and for how long you have lived in the state, with your date of birth if you want it included.
The home itself
The type of residence and the names of other residents, if you choose to give them.
Your own words where you need them
More about the purpose and any additional statements are written exactly as you type them.
Blanks, not guesses
A date you are unsure of becomes a ruled blank, and a notary block closes the page.
A lawyer reviews it before it is used
Your answers are prepared into a draft and reviewed by a qualified lawyer, rather than handed back to you as a finished form.
About this document
An affidavit of residency is a written statement, made under oath, of where a person lives and how long they have lived there. People are asked for one when they enrol a child in school, apply for a driver's license or benefits, register to vote, deal with tax or insurance, or settle something after a death.
This builder starts from what the affidavit is for, then asks for your name and address, whether it is for you or for someone else, since when and for how long you have lived there, the type of residence and any other residents. For someone else it asks for their details and your relationship to them; for a person who has died, their last address and date of death. A date you are unsure of becomes a blank line to fill in by hand.
The document writes only what you give it and never states a fact for you. It ends with a notary block. Who accepts an affidavit, and what it must say, depends on the office asking and on your state, so check with them first and ask a lawyer if you are unsure.
