Key features

  • Names the document being revoked

Blanks for the date of the power of attorney, any other agent or alternate named in it, and where it was recorded or lodged with any reference number.

  • Tells the agent to stop and hand things back

The agent is told to take no further step in your name and to return original documents, cards, keys, records and property.

  • An account of what was done

You can ask for a written account of what the agent did, with the records they hold, by a date you fill in.

  • A list of who is being told

Space for each bank, institution, office and person receiving a copy, with addresses, so reliance on the old document can end.

  • Whether a new agent is appointed

A line saying whether someone else is being appointed and, if so, who and by which new document.

About this document

A revocation of power of attorney is the written step that takes back an authority you gave to someone to act for you. It identifies the power of attorney being withdrawn, says from when the agent must stop, and lists who is being told so that banks and offices stop relying on it.

This is a blank form completed by hand. It is not enough on its own to be sure an agent can no longer act: how a revocation must be signed, delivered, recorded or notified, and what happens to people who relied on the old document, varies by state and by the document being revoked. A lawyer can tell you what yours needs.

Questions

About the revocation of power of attorney

  • Not necessarily. How a revocation must be made, who has to be told and whether anything has to be recorded depends on the state and on the original document. The form helps you record the revocation and list who is told; a lawyer can confirm the rest.
